The Ratio of Thyroglobulin in Wash-Out Fluid From Fine-Needle Aspiration to Serum Thyroglobulin Level in the Evaluation of Metastatic Cervical Lymph Nodes in Patients With Papillary Thyroid Cancer

چکیده
Abstract Fine needle aspiration with cytology analysis and thyroglobulin level of the aspirated biopsy (FNAB-Tg), is an important tool to assess metastasis cervical lymph nodes (CLN) in patients papillary thyroid cancer (PTC) who have suspicious node features on ultrasound. Despite great diagnostic accuracy FNAB-Tg, studies failed define best cutoff value for FNAB-Tg. In our study, we describe a novel way performing Tg washout process it locally at laboratory (tertiary care center). We used FNAB-Tg serum (FNAB-Tg/ serum) ratio help overcome heterogeneity both values assays detect level. conducted retrospective 22 PTC or suspected patients, All underwent fine-needle aspiration, from CLNs. was obtained all subjects using following method: Blood samples are drawn patient’s peripheral vein placed two yellow top tubes (3 ml blood each tube). 1 normal saline (NS) added Tube # (Tg Serum The aspirate via US-guided FNA. It washed NS tube #2 (FNAB-Tg Both sent local assay. FNAB-Tg/ calculated. If FNAB-Tg/Serum-Tg ratio>3, this suggestive CNL metastasis. compared results histopathology reports after neck dissection.59% (13/22) had consistent metastatic PTC. 12 out these 13 FNAB-Tg/serum >3 one TG < 3, though 4474 ng/ml 2444 ng/ml. Metastatic CLNs confirmed pathology report total thyroidectomy dissection. Six studied (27%) negative > 3. 5 dissection institute One patient elected surgery his home country not available time. Three (14%) <3. 2 them final concordant results. method differences measure FNAB. An FNAB–Tg/serum more accurate than detecting (PTC). 27% CLN would been missed if FNAB alone. This will optimize surgical approach before initial recurrence.

Diagnostic Performance of Thyroglobulin Value in Indeterminate Range in Fine Needle Aspiration Washout Fluid from Lymph Nodes of Thyroid Cancer
PURPOSE The purpose was to compare the frequency of metastatic and nonmetastatic lymph nodes diagnosed by fine needle aspiration biopsy cytology (FNAC) and thyroglobulin concentration from fine needle aspiration biopsy washout fluid (FNAB-Tg) in an indeterminate range (0.2-100 ng/mL), and to evaluate the most appropriate threshold value of FNAB-Tg in an indeterminate range. متن کاملDiagnostic Benefit of Thyroglobulin Measurement in Fine-Needle Aspiration for Diagnosing Metastatic Cervical Lymph Nodes from Papillary Thyroid Cancer: Correlations with US Features
OBJECTIVE Our goals were to determine the added value of fine-needle aspiration biopsy (FNAB)-thyroglobulin (Tg) measurements over FNAB-cytology alone for diagnosing metastatic nodes, and to determine whether the ultrasound features of lymph nodes can be used to identify lymph nodes that may benefit from FNAB-Tg measurement in patients with papillary thyroid cancer. MATERIALS AND METHODS We r...
متن کاملInfluence of thyroid gland status on the thyroglobulin cutoff level in washout fluid from cervical lymph nodes of patients with recurrent/metastatic papillary thyroid cancer.
BACKGROUND The influence of serum thyroglobulin (Tg) and thyroidectomy status on Tg in fine-needle aspiration cytology (FNAC) washout fluid is unclear. METHODS A total of 282 lymph nodes were prospectively subjected to FNAC, fine-needle aspiration (FNA)-Tg measurement, and frozen and permanent biopsies. We evaluated the diagnostic performance of several predetermined FNA-Tg cutoff values for ...
متن کاملThyroglobulin detection in fine-needle aspirates of cervical lymph nodes: a technique for the diagnosis of metastatic differentiated thyroid cancer.
BACKGROUND Fine-needle aspiration cytology is frequently used for differential diagnosis of neck masses of unknown origin. Inconclusive and even false-negative results are not uncommon. AIM To evaluate the utility of thyroglobulin (Tg) measurement in fine-needle aspirates (FNA-Tg) for detecting cervical lymph node (CLNs) metastases from differentiated thyroid carcinomas. METHODS An ultrasou...
متن کاملThyroglobulin measurements in washout of fine needle aspirates in cervical lymph nodes for detection of papillary thyroid cancer metastases.
OBJECTIVE The aim of this study was to evaluate the accuracy of the measurement of thyroglobulin in washout needle aspiration biopsy (FNAB-Tg) to detect papillary thyroid cancer (PTC) metastases. SUBJECTS AND METHODS Forty-three patients (51.4 ± 14.6 years) with PTC diagnosis and evidence of enlarged cervical lymph nodes (LN) were included. An ultrasound-guided fine-needle aspiration of suspi...
